NHS care and the immigration health surcharge
What the surcharge gives you and what you may still pay for.
The short answer
Most people who apply to come to the UK on a visa pay the immigration health surcharge (IHS) as part of the application. Once your visa starts, paying it gives you access to NHS services without a charge at the point of use, in the same way as most residents. It must be paid before your visa starts. Some services, such as prescriptions in England, dental treatment and eye tests, are still charged for.
What the surcharge covers
- Most NHS care, such as GP appointments and hospital treatment, free at the point of use.
- It does not cover everything. Prescriptions, dental work, eye tests and fertility treatment can still carry a charge.

If you have not paid yet
If the Home Office has not yet decided your application, or you have not paid the surcharge, you may be charged for NHS treatment. Emergency treatment is a separate question, so check the NHS guidance for your situation.
Proving your status
You may be asked to prove your status when you use the NHS. If you have a digital immigration status (an eVisa), you may need to use a share code to show it.
Practical steps
- Register with a local GP when you arrive, so you have one before you need one.
- Keep your visa documents, your IHS payment confirmation and your share code details safe.
- Ask about costs before treatment if you are not sure whether you will be charged.
